Once a contact form is submitted, the family will be called to set up an appointment. A non-refundable fee of $50 is required to secure the appointment time and will be subtracted from the total cost of the appointment. A link will be sent to the designated parent/caregiver to establish access to the parent portal of the EHR. The intake packet, consents, and authorizations must be completed through the parent portal prior to the visit. All communications will be through the parent portal of the EHR.
